For three hundred years it meant any fuzzy thing in the sky nobody could resolve. Andromeda was filed as a nebula until 1925, when Hubble worked out it was an entire separate galaxy, two and a half million light years out, containing a trillion stars.
It was always that. We just did not have the lens yet.
